ASSUMPTION OF RISK AND WAIVER OF LIABILITY

I acknowledge that participation in ocean safety training and freediving activities involves inherent risks, including but not limited to shallow water blackout, drowning, physical injury, and other water-related hazards.

I voluntarily assume all risks associated with participating in FreediveSafe! Hawaii training programs and activities. I understand that FreediveSafe! Hawaii, its instructors, volunteers, board members, and affiliates cannot guarantee my safety and are not responsible for accidents or injuries that may occur.

I hereby release, waive, discharge, and covenant not to sue FreediveSafe! Hawaii, its officers, employees, instructors, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me while participating in such activities.

I certify that I am physically fit and have no medical conditions that would prevent safe participation in ocean safety training. I agree to follow all safety protocols and instructions provided by FreediveSafe! Hawaii instructors.

I give permission for FreediveSafe! Hawaii to use photographs or videos taken during training events for educational and promotional purposes.
